BRADYCARDIA

Bradycardia represents a heterogeneous group of sinus node and atrioventricular conduction disorders resulting from impaired impulse formation and/or propagation. Clinical relevance is determined by the underlying substrate, degree of conduction system disease, and associated hemodynamic impact, with advanced bradyarrhythmias contributing to syncope, heart failure syndromes, and malignant bradyasystolic events.

2021 ESC guidelines on evaluation of the patient with suspected or documented bradycardia or conduction system disease3

A careful history and physical examination are essential for the evaluation of patients with suspected or documented bradycardia.

Current guidelines emphasize the importance of the history and physical examination in the initial evaluation, particularly for identifying patients with structural heart disease.

Optimal pacing mode and algorithm selection in sinus node dysfunction (SND) and atrioventricular (AV) block:

a(R) indicates that the programming of such a pacing mode is preferred only in the case of chronotropic incompetence.
